Geriatric Care Manager specializes in offering older adults, the handicapped, the memory impaired and persons overcoming a major surgery a wide range of services including developing a plan for continuing care, assistance of the families in choosing the services that are most appropriate. The Geriatric Care Manager arranges services, monitors, and establishes a relationship with client and service provider. He or she counsels families, assists in relocating client and setting up arrangements and is there when a crisis arises. Their goal is to bring support to all families experiencing the challenges that come with aging.

These services may include: selecting a senior residence such as community care retirement living, assisted living, assisted living for memory impaired, personal care homes, or choosing in-home care, home healthcare for occupational therapy and physical therapy, respite care, hospice care, also provides educational information on financial advisement, eldercare questions, financial aid and many more options for seniors.
