Geriatric Care Manager specializes in offering older adults, the handicapped, the memory impaired and persons overcoming a major surgery a wide range of services including developing a plan for continuing care, assistance of the families in choosing the services that are most appropriate. The Geriatric Care Manager arranges services, monitors, and establishes a relationship with client and service provider. He or she counsels families, assists in relocating client and setting up arrangements and is there when a crisis arises. Their goal is to bring support to all families experiencing the challenges that come with aging.
